On February 28, ten years before this talk, don Miguel Ruiz survived a massive heart attack. Rather than viewing it as a near-death trauma, he frames the experience as a gateway to his most important teaching: how to surrender to death, let go of the physical body, and recognize that what we truly are—consciousness, life force itself—cannot die. How Did a Heart Attack Become a Teaching Moment?

Don Miguel Ruiz's heart attack struck at 4 a.m. on February 28, roughly a decade before this talk. He felt intense chest pain and immediately recognized what was happening. Rather than panic, his first response was practical: he called 911 and walked downstairs to meet the ambulance in the living room. When his mother asked what was wrong, he calmly told her, "Don't worry, it's just a heart attack." His equanimity in that moment wasn't denial—it was clarity. He knew he was dying, and he was prepared to use that knowledge as his final teaching.

Once in intensive care, surrounded by tubes and machines, Ruiz made a deliberate choice about how to spend what he believed would be his last hours. He arranged his legal affairs, ensured his loved ones were informed, and waited for opportunities to share what he had learned. The heart attack, in his view, was not a setback or tragedy—it was an opportunity to embody and transmit the deepest wisdom he had spent decades teaching.

What Does It Mean When the Physical Body Dies?

When Ruiz's middle son Jose rushed into the intensive care room, he was devastated. Seeing his father in a hospital bed with medical tubes, close to death, Jose broke down crying: "Father, please don't die. It's not time. I'm not ready." Ruiz's response was not comforting in the conventional sense. Instead, he told his son to leave the room, collect himself, and return when he was emotionally stable. Jose was shocked, but he left and came back composed.

Once Jose returned, Ruiz delivered the core of his teaching about death and identity. He explained that what will die is only the physical body. "I will not die," he said. "I still live in the memory of everybody who loves me and everybody who knows me and through my books to many many people who don't even know my physical body." Ruiz pointed out that what his son knows as "father" is not the physical form lying in the hospital bed. It is an image in Jose's mind—a perception built from interactions, words, and the son's own interpretation.

This distinction is crucial. We do not actually know anyone as they truly are; we only know the image we have constructed in our minds. "You have no idea what I have in my mind, not even in your dream. Well, it's exactly the same thing. You see anybody around you and you have no idea what they have in their mind." When the physical body dies, the image in the mind does not automatically die with it. It persists in memory, in the influence of his books, in stories and teachings shared. The relationship continues in dreams and imagination.

How Does Consciousness Persist After Death?

Ruiz taught his son a radical reframing of what death means in practical terms. If Jose dreams of his father after his physical death, the dream will feel as real as waking conversation. In the dream, Ruiz can appear, speak, and interact. Jose would not know whether his father is "really" there or whether it is only his imagination. In fact, Ruiz suggests, there is no difference. The distinction between an image in the mind and a "real" presence dissolves when we recognize that all perception is mediated through consciousness.

This is not wishful thinking or fantasy. Ruiz is describing the actual mechanics of how we experience any other person. "When you're dreaming, just talk to me and I will answer you. Because even that I am just an image of what you perceive, in the end, we are exactly the same thing. There is no difference between you and I, and there is no difference between everybody else and myself."

Ruiz shared that he himself still speaks with his own father in dreams, and his father answers. These are not conversations with a ghost or supernatural being; they are conversations in which Ruiz is speaking to the image of his father that exists in his own mind—which is, in Ruiz's view, the same as speaking to his father directly, because the separation between self and other is illusory.

Why Do We Suffer When Loved Ones Die?

Ruiz identified the root of death-related suffering: attachment and the fear of not seeing loved ones again. "Not because you're dead, because they will not see you again." The pain of grief is not actually caused by the beloved's death—it is caused by the fear of abandonment, of missing their presence.

He offered a compassionate observation about those dying from chronic illness. "Just imagine all that people who are condemned to die and they're suffering a lung disease with a lot of pain and they not go because everybody have a big attachment on them and they're so afraid to die. That can be changed if we have awareness." People cling to life not because life is inherently precious, but because others are attached to them and they do not want to cause pain. This creates prolonged suffering on both sides—the dying person in physical agony, the family in emotional anguish. If the family understood death differently, if they could celebrate the end of suffering rather than mourn the loss of form, the dying could release their hold more peacefully.

What Is the Difference Between Death and Defeat?

Ruiz made a philosophical distinction that recasts mortality entirely. "Everything that is created has an end. But the one who create is immortal. And the one who create is life itself. I am life. You are life. And everything that you perceive is alive because life is the force that is moving matter. And we are exactly the same force."

The implication is radical: there is no separation between the individual and the universal life force. Death is not the end of "I" in any ultimate sense. What ends is a particular form, a particular expression of the eternal force of life. The body returns to matter; the life animating it returns to the universal field from which all forms arise.

When Ruiz said to his son, "We conquer death. Leaving my physical body doesn't mean that I was defeated by death. No the opposite. I let go my physical body," he was describing a shift from resistance to surrender. Most people experience death as something that happens to them, an external force stealing their existence. Ruiz reframed it as a choice—a letting go that is an act of power, not defeat. "That's what make me feel so excited because my body will feel when I live and that will be the last thing that my body will remember me life."

How Should We Celebrate the Death of Others?

Ruiz repeatedly used the word "celebration" to describe the proper response to death. When Jose first came to his bedside in tears, Ruiz told him, "This is the way you're going to celebrate the death of your father. Get out of the room, fix yourself." Later he emphasized: "This is for celebration, not for crying." And: "The way you're going to celebrate the death of your father because it is a celebration."

This is not callousness. Ruiz acknowledged that he had pain in his chest. He was not denying the physical reality of heart attack or the emotional response of loved ones. But he was asking his son to shift from identifying with the emotional drama to recognizing the metaphysical reality. A father is not his body. The continuation of the father's presence in the son's mind, memory, and dreams—and in the teachings he has left behind—is the real inheritance. To honor that inheritance, the son should not remain trapped in grief but should move toward understanding, toward joy at the release from suffering, toward recognition of unity.

What Happens When We Accept That We Are All One?

At the end of his teaching to Jose, Ruiz said, "Now you know that we all are only one living being. And the force of moves everybody is exactly the same force. Now as you can see we conquer death." This is the crux. If there is truly no separation between persons—if we are all expressions of a single life force—then death of any individual form does not separate us. The other continues to exist as part of the one being that contains all consciousness.

Jose grasped this. He said to his father: "Whatever is in the mind of your image, in my dream, in my imagination is really my own. I am myself." He realized that the conversation he was having with his father, and any future conversations in dreams, would be conversations with himself—which meant they would also be conversations with the universal consciousness expressing itself in both father and son.

Ruiz confirmed Jose's understanding: "Yes you got it. It's wonderful." This recognition is the antidote to existential terror. If the boundary between self and other is ultimately illusory, if all minds are expressions of a single awareness, then no one ever truly dies and no one is ever truly alone. The physical separation is real, but it does not constitute a break in fundamental unity.

How Did Recovery Change Ruiz's Life After the Heart Attack?

Ruiz's recovery from the initial heart attack was not immediate. The physical damage was severe. It took a full year of rehabilitation before he received clearance from doctors to resume his teaching work. During that time, he confronted not only mortality but the question of whether his body could still function as an instrument for his purpose.

When he was told he would need bypass surgery, he approached it with the same equanimity he had brought to the initial crisis. "The surgery is great because I will not have the fear of heart attack. It's a great experience." He saw the surgery not as a defeat but as an opportunity to renew his physical capacity.

After recovery and surgery, he resumed traveling and teaching. He went to Costa Rica and other places to continue sharing his work. But the near-death experience had fundamentally altered his relationship to his body and his mission. He was no longer teaching from intellectual understanding alone; he was teaching from direct, embodied knowledge of death's approach and the peace that comes from accepting it.

What Is the Key to Changing Your Experience of Reality?

In the final portion of the transcript, Ruiz pointed to the mechanism of transformation: "The key to change your world is to change what you believe you are, which means what you believe you are." Our perception of reality is not objective; it is generated by belief. We create a world in our minds, and that mental world is the world we actually inhabit. The physical world exists, but the meaning we assign to it, the identity we claim within it, and the story we tell about it are all products of belief.

If someone believes they are a temporary, isolated ego that will cease to exist at death, they will live in fear and grasping. If someone believes they are an eternal expression of universal life—capable of dying in form but never in essence—they will live with freedom and gratitude. The external facts of mortality remain the same; the internal experience transforms completely.

[0:11] and last for me.

[0:14] I celebrated

[0:17] that 10 years ago in February 28

[0:23] at 4 in the morning

[0:26] I have a massive heart attack.

[0:32] 10 years ago.

[0:35] As you can see,

[0:38] I survived this heart attack,

[0:41] but it was a little traumatic.

[0:45] That strong pain in my chest woke me up

[0:51] and I knew right away that I was having

[0:54] a heart attack.

[0:58] Even that the pain was

[1:01] very strong.

[1:03] I feel so excited

[1:07] because finally

[1:10] I was very close

[1:13] to have the greatest experience of my

[1:17] life.

[1:20] And that experience of my life perhaps

[1:23] is living my physical body.

[1:27] And I had the great opportunity

[1:31] to share to everyone

[1:36] the best way how to let go the physical

[1:40] body.

[1:44] Anyway,

[1:47] first thing it was to take the telephone

[1:51] marked 911

[1:55] and let them know the symptoms.

[2:00] Once I hang up,

[2:04] I decide to walk down the stairs because

[2:08] it was a a house with two floors

[2:12] that I can wait for them in the living

[2:14] room

[2:16] going down from the stairs.

[2:20] My mother who was living with me at that

[2:22] time,

[2:25] she heard noise.

[2:27] She stand up.

[2:29] She saw me and say, "What's going on,

[2:31] Miguel?"

[2:33] And I say, "Sarita, because this is how

[2:36] I used to call my mom."

[2:38] Don't worry, it's just a heart attack.

[2:43] Well, as you can see, she got so scared

[2:47] right away.

[2:50] And I say, "Don't worry. I already

[2:52] called uh uh 911 and they will be here

[2:56] at any moment. just please call my

[2:59] brothers, let them know what is going on

[3:03] and just let let everybody else know

[3:06] that I will be in the hospital and give

[3:10] me time to call um my manager that at

[3:13] that time was Stephanie Bureau

[3:17] and let her know what's going on and to

[3:21] let everybody know that I was going to

[3:24] the hospital

[3:27] and that I I'm so excited because I may

[3:29] share perhaps my last teaching.

[3:34] How to surrender to death, how to let

[3:37] go, and even more how to enjoy it.

[3:44] The ambulance come very soon

[3:48] and

[3:50] just in a few minutes I was already

[3:54] in the emergency room in the hospital.

[3:57] to be translate a little later

[4:01] to the unity of intensive care.

[4:09] You know, when you are in a hospital

[4:11] with a heart attack, it's hard to

[4:13] measure the time.

[4:16] At the same time, it goes very fast and

[4:18] very slow.

[4:21] I asked right away

[4:25] to bring whoever I need to make the last

[4:28] arranges of my life because I knew for

[4:32] sure that I was dying. I have no doubt

[4:34] in my mind because this is one of the

[4:36] biggest symptoms of the heart attack.

[4:40] Then very soon Stephanie were there. I

[4:43] asked bring a lawyer that tried to

[4:46] restrain my life and leave everything to

[4:48] my children etc etc.

[4:52] Anyway,

[4:54] I was in intensive care

[4:57] when my middle son goes

[5:01] he came to see me.

[5:05] I saw him coming in and I was so happy

[5:08] to see him.

[5:11] But then I noticed that he was crying

[5:12] and he started talking, "Father, please,

[5:14] please don't die. It's not time. I'm not

[5:17] ready.

[5:18] You have to stay a little longer for me.

[5:21] Don't go." He was crying.

[5:25] It was the very first time that he saw

[5:27] his father we waking in a bed with all

[5:31] those tubes

[5:34] and close to death.

[5:39] I stand up a little and look into his

[5:41] eyes and say, "Jose,

[5:46] this is the way you're going to

[5:47] celebrate the death of your father.

[5:51] Get out of the room,

[5:53] fix yourself, and when you're ready,

[5:56] come back because I have something

[5:58] important to share with you."

[6:02] You can imagine he was in a little

[6:04] shock.

[6:06] He left and a few minutes later he came

[6:09] back.

[6:12] When he came back it was completely

[6:14] different.

[6:16] He say father I am sorry.

[6:20] I notice how selfish I am

[6:24] to take the last moments of your life

[6:27] with all my pity,

[6:29] all my emotional pain. And you're not

[6:32] even dead yet.

[6:35] I have a big smile and and I tell him,

[6:38] "Well, this is for celebration,

[6:41] not for crying." And yes, I have pain,

[6:45] but I'm happy and I'm ready to go.

[6:49] There's not a problem at all.

[6:52] And by the way, what will die is my

[6:56] physical body.

[6:59] But I will not die.

[7:02] I still live in the memory of everybody

[7:06] who loves me and everybody who knows me

[7:12] and through my books to many many people

[7:17] who don't even know my physical body

[7:20] because I put myself in those books

[7:26] in any way and everybody else's mind I

[7:30] only a

[7:32] I am the image that they perceive about

[7:34] me because they really don't know me as

[7:38] I don't know them.

[7:42] Then the father that you know is only in

[7:44] your mind and is an image of this

[7:46] physical body that is dying right now.

[7:50] But because the physical body is dead,

[7:52] it doesn't mean that my image in your

[7:54] mind is also dead.

[7:58] If you notice when you dream with me or

[8:01] about me,

[8:03] you have no idea what I have in my mind,

[8:06] not even in your dream.

[8:09] Well, it's exactly the same thing. You

[8:11] see anybody around you and you have no

[8:12] idea what they have in their mind.

[8:16] Then the only thing that you can have to

[8:18] do is when you're dreaming, just talk to

[8:21] me and I will answer you.

[8:26] Because even that I am just an image of

[8:28] what you perceive.

[8:30] In the end, we are exactly the same

[8:33] thing. There is no difference between

[8:36] you and I. And there is no difference

[8:38] between everybody else and myself.

[8:42] We all the same. We're just not aware of

[8:45] that.

[8:47] That's the way I talk to my father when

[8:50] I am sleeping.

[8:53] And he answered me. I play with him. All

[8:55] my dreams that I have with my father is

[8:58] all about joy.

[9:01] I never suffer because he died.

[9:05] Perhaps I was feeling a little a little

[9:08] emotional pain when I see him dying.

[9:12] But it was a big relief when he finally

[9:15] stopped suffering.

[9:17] Now just imagine that you are the one

[9:20] who's dead and you're in the coffin

[9:23] and you can see all your family crying

[9:25] for you and making a big trauma because

[9:28] you are dead

[9:31] and then in your point of view you will

[9:34] say well I have no pain I'm so happy

[9:36] there is no way I want to go back to

[9:38] that body

[9:40] but you see everybody crying because you

[9:43] are dead

[9:46] they are crying them because they know

[9:49] they will not see you again.

[9:53] Not because you're dead, because they

[9:54] will not see you again.

[9:57] Just imagine all that people

[10:01] who are condemned to die and they're

[10:02] suffering a lung disease with a lot of

[10:04] pain and they not go because everybody

[10:08] have a big attachment on them and

[11:22] because whatever is in the mind of your

[11:25] image, in my dream, in my imagination

[11:32] I have a base say yes you got it. It's

[11:36] wonderful.

[11:38] Now you know that we all are only one

[11:40] living being.

[11:43] And the force of moves everybody is

[11:45] exactly the same force.

[11:47] Now as you can see

[11:50] we conquer death.

[11:54] Then leaving my physical body it doesn't

[11:58] mean that I was defeated by death. No

[12:00] the opposite. I let go my physical body.

[12:05] And that's what make me feel so excited

[12:27] and I tell not did you understand he say

[12:30] yes father he say now you can go out

[12:33] because there's many other people

[12:34] waiting for me

[12:37] and he left.

[12:41] But for me, this moment was extremely

[12:44] important because I couldn't share that

[12:48] experience with all the people that I

[12:50] wish number one because there was

[12:53] impossible that they come to see me in

[12:54] intensive care

[12:57] and that the doctors allow them to come.

[13:01] But in my imagination when I have the

[13:03] heart attack in the beginning, I imagine

[13:06] myself telling the whole world and

[13:08] everybody

[13:09] what I was sharing with my son to Jose

[13:13] when my son Mikuel Jr. came.

[13:16] I'm not really myself anymore. I barely

[13:20] remember when he come to see me because

[13:24] when he come to see me, I didn't know I

[13:27] have a heart attack. I was already

[13:30] dreaming and I thought that he was in

[13:33] his way

[13:35] to Baja California south to see his

[13:39] girlfriend

[13:41] and I thought he just stopped just to

[13:43] see me and say, "Father, I love you. I

[13:46] going to see my girlfriend and I see you

[13:48] later." I give you all my blessings.

[13:52] And after that, I went into coma for

[13:55] nine weeks.

[13:58] nine weeks in coma.

[14:02] When I came out of the coma, the doctors

[14:06] let me know

[14:08] that I was living only with the 16% of

[14:10] my heart capacity.

[14:14] And what they recommend

[14:17] is that I spend the rest of my life in a

[14:20] sofa watching TV

[14:24] or reading books.

[14:27] When I was at home and I talked to my

[14:30] children,

[14:31] I told them there is no way that I will

[14:34] stay at home waiting for death.

[14:37] No, I will go out and I will look for

[14:41] death by doing what I love to do with my

[14:45] life.

[14:46] to follow my passion

[14:49] which is to share the message that I've

[14:52] been sharing for more than 20s something

[14:54] years

[14:56] to let everybody know that everything is

[14:59] possible if this is what they really

[15:02] want to do that the only thing that they

[15:05] need

[15:06] is a strong will and discipline and with

[15:11] that they can make their dreams comes

[15:14] True.

[15:19] The doctors told me

[15:21] that I may survive one maybe two years.

[15:29] Then I start planning everything for

[15:33] my future debt. Whenever it happen

[15:35] doesn't matter.

[15:40] But the only problem that is that I was

[15:43] living in extreme pain all the time. My

[15:47] body was always in pain.

[15:50] But it was not a justification

[15:52] to suffer

[15:54] because I really enjoy life.

[15:58] And hardly anybody noticed that my body

[16:00] was in pain because I keep that

[16:03] wonderful smile completely sincere and I

[16:07] really enjoy everything that I can

[16:09] perceive.

[16:11] I enjoy the activity as I enjoy the

[16:15] rest.

[16:20] And for eight and a half years I was

[16:22] living in that condition.

[16:25] And every day it was getting a little

[16:27] worse and a little worse. Then but by 19

[16:32] uh 2009 in August

[16:36] I decide that because I didn't die this

[16:38] two years.

[16:41] I will try to have a heart transplant. I

[16:45] apply. It took a whole year for the

[16:48] doctor to test all my physical body in

[16:52] order to allowed

[16:55] to have the heart transplant. And they

[16:56] put me in the list in August in 2010.

[17:00] And then they told me, well, uh, you may

[17:04] have the heart transplant between 6

[17:06] months and one year.

[17:09] But I knew that it will be sooner. I was

[17:11] thinking maybe by November I may have my

[17:15] new heart.

[17:17] And I tell that to myself because I was

[17:19] really feeling that I may not survive

[17:23] the end of the year 2010.

[17:29] Then in October 9,

[17:33] 2010,

[17:34] they call me because I was doing uh a

[17:38] lecture in Texas with my son to Miguel

[17:40] Rich Jr.

[17:43] They call me at midnight and they say,

[17:46] "We have a heart for you. You have to be

[17:49] here in the hospital in around three

[17:52] hours

[17:53] from Texas to LA in three hours."

[17:58] I say I will be there.

[18:02] Well, it took me six hours to arrive

[18:07] to Los Angeles

[18:10] and get to the hospital

[18:13] where the heart was waiting for me.

[18:16] Before I leave Texas, I tell my older

[18:19] son because in that moment say, "Okay, I

[18:22] will go with you." I say, "No, you have

[18:25] a work to do.

[18:27] You have to finish your work.

[18:30] This is not an excuse for you to to stop

[18:32] doing what you're doing, to enjoy what

[18:34] you're doing

[18:36] because you have to understand that for

[18:37] me it's a win-win situation. I have

[18:41] nothing to lose. I'm not afraid at all

[18:46] to that surgery.

[18:48] Let's see that I don't wake up from the

[18:50] surgery is great because I will not have

[18:53] that pain any longer.

[18:56] And if I wake up from the surgery, that

[18:59] means that the whole process procedure

[19:02] was a success.

[19:06] And then I will experience what it means

[19:08] to live with somebody else's heart,

[19:12] which is a great experience. Can you

[19:15] imagine? live with somebody else's heart

[19:20] to come back to life

[19:24] and that you can see that you can enjoy

[19:26] even more every single moment of your

[19:28] life.

[19:32] As you can see

[19:34] the surgery was a success

[19:39] and my recovering is been we can call it

[19:42] a miracle.

[19:45] I never have not a little a little

[19:48] symptom of rejection

[19:52] and I get stronger and stronger that in

[19:54] September last year they give it the

[19:59] green light that I can go back and do my

[20:03] work.

[20:05] And since September until now, I really

[20:08] went three times to Canada, like another

[20:12] two, three times to Mexico, to different

[20:15] states in United States. I went to

[20:18] Japan,

[20:19] I went to Costa Rica, I went to

[20:22] Colombia,

[20:25] I went back to Mexico once again.

[20:29] And the whole calendar 2012 is

[20:32] completely full.

[20:37] Today

[20:38] I celebrate

[20:42] my dead day,

[20:45] 10 years of the heart attack.

[20:50] And once again,

[20:52] I ask all of you that please help us to

[20:57] change the world.

[21:01] Not the planet earth,

[21:03] not humanity,

[21:05] the world that you create in your own

[21:08] head.

[21:10] Because only you can change it because

[21:13] only you know what is what you like and

[21:16] what is what you don't like. Only you

[21:18] know what make you happy and what make

[21:21] you unhappy.

[21:24] Then the key to change your world is to

[21:27] change the main character of your story

[21:30] which means yourself.

[21:33] If you change any anybody else it didn't

[21:36] make a change in your world. It it

[21:39] didn't make a difference.

[21:41] Then if you change the main character

[21:45] which means what you believe you are

[21:48] just like magic everything around you

[21:50] start changing.

[21:53] And the benefit after your own is your

[21:59] beloved, the one that you live with,

[22:03] your children,

[22:05] your parents, your brothers, your

[22:06] sisters,

[22:10] your family,

[22:12] your friends, your community,

[22:16] your country, and finally the entire

[22:20] humanity.

[22:24] Then please help us to change the world

[22:28] by changers your own.

Ruiz teaches that all forms are expressions of a single universal life force. The boundaries between individuals are real at the physical level but illusory at the level of consciousness. Recognizing this unity dissolves the existential isolation that creates fear of death and suffering in grief.
We create our world through belief. What you believe about your identity, mortality, and your place in existence determines the meaning and experience you have of those things. Changing belief is the key to transforming your lived reality.
